Yeah, the devil horns make it pretty clear to me that this is just Heady choosing to appear this way, not a reflection of actual (Head) Alien anatomy.
Took me a second to figure out what was going on in the first five panels. I initially thought it was like one of those scenes in Dragon Ball where the heroes convince the villains to fight them in some abandoned wasteland without any people, but it’s actually the exact opposite– Head Alien teleporting everybody else away.

Head Alien wants to make sure he has an audience consisting of only the people he’s about to destroy.
“For what you’ve done to me”, said the alien to the people he abducted and experimented on when they were toddlers.
“Do you know what you did? YOU GOT REVENGE ON ME FOR MY CRIMES COMMITTED AGAINST YOU”
Really, HA’s always been one of those you-made-me-hurt-you victim-blaming parents.
Did HAII teleport everyone away, or just a few of them? I can’t tell.
He teleported everyone except Joyce, Walky, and Jason away— the three people he specifically has a grudge against.
