The implication is that Joyce either had a cowboy hat lying around or went out and bought one specifically for this purpose.
Either way raises several questions.
All women in their 20’s had at least one cowboy hat in the early 2000s. It was like a law or something.

My grandmother gave it to me. I’m… not really sure why.
I didn’t keep it though – she had the receipt, and the leather shop where she bought it also had leather pants, so… exchanges were made.
Who else would give Joyce that book?

I am unsure about whether she is joking, or honestly misunderstanding.

That brings up an interesting question.
When Joyce and Walky first had sex, they wrecked a hotel room. Are we to assume they did so purely in basic missionary? Or did they try out lots of different positions?
Depending on those assumptions, the joke here is either that they have tried many positions but don’t know the proper names OR that they do know the proper names and Joyce is making a lewd suggestion here. Either one is funny, but they’re funny in entirely different ways based on assumptions.
… the idea that they’ve only ever done it in missionary, on the other hand, is less funny than it is sad.
Maybe she bought it herself?

Was that an illustrated book? Were there cowboy hats in the illustrations? Did she steal one of Sal’s books?
